   Crisis Prevention and Crisis Intervention

   Comprehensive Assessments based on medical, financial, psychological and safety status

   Verbal and written communication with family members and/or responsible parties for status updates & prior authorization for       recommended services

   Assistance with lifetime legal documents

   Client Consultation, Testing and Counseling

   Cost Analysis Projections with options and recommendations provided

   Referrals to Geriatric Physicians, Elder Law Attorneys and available community resources

   Hospital, Nursing Home, Assisted Living Facility, and In-Home Visitation

   Assistance with transfers to alternative living settings

   Coordination and supervision of licensed staffing including R.N.’s, L.P.N.’s, H.H.A.’s in facilities or in home settings

   Negotiation of cost effective rates for all services with utilization of Medicare, Medicaid and insurance benefits

   Complete medical management includes scheduling and attendance to physician appointments, consultation with physicians,       monitoring of reactions to changes in orders and appropriate follow-up care.

   Liaison between family, physicians and significant others as appropriate

   Complete medication management per physician orders

   Complete dietary management including documented intake and output

   Monitoring for ancillary needs such as optometry, dental, otolaryngology

   Assistance with instrumental activities of daily living such as bill paying, mail sorting, social calendars, transportation

   Legal documentation of nursing & care management progress notes

   Assistance with yearly guardianship plans

   Assistance with preparation of Advanced Directives

   Bereavement Counseling including assistance with funeral arrangements
